No, SEM will only filter incoming e-mails and sort out incoming spam/malware. Filtering outgoing spam/malware through Microsoft Exchange server doesn't really make any sense in real life scenarios. Today's spam/malware is practically always sent out through malware installed on a client machine - malware which doesn't use an Microsoft Exchange server to send out spam/malware but instead uses its own built in SMTP client.
Should SEM filter outgoing e-mails it would just use more resource on the server without providing any additional security in regards to spam/malware.
If you believe there are spamming activities within your organization, we suggest using a firewall for blocking any e-mail sending from within your organization and configuring the firewall to only allow the Exchange server to send e-mails.
